The medical Examination Report Form D4 has to be completed by: Anyone who is making their first application for a licence to drive a vehicle over 3.5 tonnes or a public service vehicle with more than 8 seats. Anyone renewing their LGV/PCV entitlement aged 45 or over; Anyone aged 45 or over will have to renew their licence every 5 years, then every year after the age of 65. Some medical conditions will result in a licence being granted for 1, 2 or 3 years. The DVLA should give notice prior to the renewal date.

What happens at my medical? Medicals usually take 20 - 30 minutes. The doctor will complete a D4 form questionnaire with you and then carry out a detailed physical examination which includes: an eye test, urine test and blood pressure measurement.
If there are any abnormal findings the doctor will inform and advise accordingly. What is the most common reason for failing a medical? One of the most common causes of failing a medical is poor eyesight. If you think that your eyesight is not as good as it should be it may be best getting your eyes tested by an optician before the medical is due, as this could save you from failing your medical. What if the medical shows up symptoms which could indicate a serious health condition? This is a rare event. In such cases the examining doctor will give you a letter to take to your own GP asking him or her to re-examine you with respect to the initial abnormal findings and investigate accordingly.
